About this job

Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services to home health patients across Summersville, WV. The clinician will plan, implement, and evaluate individualized plans of care to restore or maintain patient function. Responsibilities include assessment, treatment of neuromuscular, neurological, and orthopedic disorders, patient and family education, safe transfer and durable medical equipment instruction, documentation of plans of care and progress notes, and collaboration with the interdisciplinary team. The role requires travel within the local service area and the ability to perform physical tasks such as lifting, bending, and prolonged walking.


Minimum Qualifications

  • Current state license as a Physical Therapist required.
  • Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an APTA‑approved program required.
  • Proof of current CPR certification required.
  • Current driver’s license, reliable transportation, and active vehicle insurance required.
  • 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
  • Ability to perform physical demands of home health visits including lifting, bending, pulling, pushing, prolonged standing, and walking.
  • Ability to read, write, and speak fluent English.
  • Willingness and ability to travel locally and float within the service area as needed.


Preferred Qualifications

  • Previous home care experience preferred.
  • Demonstrated knowledge of durable medical equipment preferred.


Assignment Details

  • Clinician will travel locally to patient homes within the Summersville, WV service area.
  • Must be able to perform home assessments and adapt equipment to home limitations.
