\n\nJob Summary\nCuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
\n\nResponsibilities\n- \n
- Perform patient care coordination and discharge planning in an acute care environment. \n
- Assess patient needs and coordinate with social services and transportation for transitions of care. \n
- Organize and participate in family meetings to develop comprehensive treatment plans. \n
- Utilize InterQual Criteria and participate in admission procedures for patients. \n
- Independently arrange for durable medical equipment, skilled nursing facility transfers, and ambulance transport. \n
- Maintain up-to-date documentation in Epic charting system. \n

- Current and valid CA state RN license; pending license accepted if licensed upon start date. \n
- Minimum of 2 years of acute care hospital case manager experience is required. \n
- Proficiency with computerized documentation and Microsoft applications. \n
- Experience with HMOs, IPAs, CMS guidelines, and discharge planning processes. \n
- Knowledge of drug/alcohol abuse treatment options and other community resources. \n
- Available for weekend rotations and able to commit to working 3 of the 5 major holidays. \n
