About the Role

Gabby Health, operating under Medical Solutions Consultants, is seeking licensed physicians, nurse practitioners (NPs), and physician assistants (PAs) to support our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs across skilled nursing facilities in Montana.

This is a flexible, per-diem opportunity where providers conduct required visits either in person at facilities or remotely via telehealth, supporting chronic condition management and Medicare-compliant care delivery

What You’ll Do

• Conduct initial CCM enrollment visits for SNF residents
• Perform in-person or telehealth assessments to establish care plans
• Review and co-sign clinical documentation for accuracy and compliance
• Document visit notes within 24 hours
• Collaborate with facility staff and care coordination teams
• Ensure patient consent and proper program enrollment documentation
• Adhere to CMS billing guidelines and Montana telehealth regulations

Requirements

• Active, unrestricted Montana license (MD, DO, NP, or PA)
• Active NPI (Type 1) and Medicare enrollment
• Experience in primary care, geriatrics, internal medicine, or long-term care
• Strong understanding of HIPAA and compliance standards

Preferred

• Experience in SNF, ALF, or post-acute care settings
• Familiarity with CCM, RPM, or chronic disease programs
• Comfortable with telehealth platforms and EHR systems
• Bilingual (Spanish/English) is a plus
