Resident Services Coordinator

What You'll Do:

• Coordinate medical, dental, therapy, and personal appointments
• Schedule and arrange resident transportation
• Escort residents safely to and from appointments
• Work closely with residents, families, care teams, and providers
• Supervise and assist the Transportation Aide
• Maintain appointment and transportation records
• Provide compassionate, resident-centered support

Qualifications:

• Current Pennsylvania CNA certification required
• Valid driver's license with clean driving record
• Strong communication and organizational skills
• Healthcare or long-term care experience preferred
• Comfortable with scheduling/documentation systems

Physical Requirements:

• Ability to lift and transfer up to 50 lbs.
• Frequent walking, standing, and wheelchair assistance
• Ability to assist residents in and out of vehicles
