Physical Therapist Assistant - Nashville, TN

All Heart Home Care is seeking a dedicated and compassionate Physical Therapist Assistant (FT) to support our home health team in the Nashville area. This position offers flexible, as-needed scheduling while providing one-on-one care to patients in their homes.

Key Responsibilities:

• Provide in-home physical therapy services under the supervision of a licensed Physical Therapist
• Assist in implementing individualized treatment plans to improve patient mobility, strength, and function
• Monitor patient progress and report changes to the supervising Physical Therapist
• Educate patients and caregivers on prescribed exercises, safety techniques, and home care routines
• Maintain accurate and timely documentation in accordance with company policies and regulatory standards
• Communicate effectively with the care team to ensure coordinated and high-quality patient care
• Travel to patient homes within the assigned service area

Qualifications:

• Graduate of an accredited Physical Therapist Assistant program
• Active Physical Therapist Assistant license in the state of Tennessee
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong interpersonal and communication skills
• Ability to work independently and manage time efficiently
• Previous home health experience preferred but not required
