As a Physician Assistant with Twin Cities Physicians, you will provide on-site care throughout senior living communities while managing chronic conditions, coordinating care, and helping patients navigate complex healthcare needs. Responsibilities

• Provide on-site rounding care throughout assisted living, memory care, and long-term care communities.

• Perform physical examinations, chronic disease management, preventative care, and follow-up visits.

• Develop and adjust individualized care plans based on patient needs.

• Collaborate with physicians, nurses, therapists, caregivers, and community staff.

• Communicate effectively with families and care teams regarding treatment plans and patient progress.

• Support continuity of care and help reduce unnecessary emergency room visits and hospitalizations.

• Complete documentation, medication management, and lab coordination.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, or post-acute care preferred.

• Strong clinical judgment, communication skills, and patient-centered approach.

• Valid driver's license and reliable transportation.
