As a Palliative Care Nurse Practitioner, you’ll join a mission‑driven team that values courage, compassion, and creativity while helping launch and grow a brand‑new palliative care program from the ground up and experience a rewarding work life while contributing to our team in a full-time, salaried position.

Responsibilities:

  • Providing holistic, patient-centered palliative care to individuals with serious illness and their loved ones in the greater Grand Rapids, MI area.
  • Conduct thorough assessments, develop and manage individualized care plans, and perform ongoing follow-up visits.
  • Lead and support symptom management (e.g., pain, dyspnea, nausea, anxiety) using evidence-based practices.
  • Facilitate goals-of-care conversations, advance care planning, and support informed decision-making aligned with patient values and preferences.
  • Communicate effectively with patients, families, caregivers, and referral sources to ensure continuity and quality of care.
  • Provide education to patients, families, and staff regarding disease progression, treatment options, and supportive care strategies.
  • Prescribe medications and therapies in accordance with scope of practice and collaborating physician agreements.
  • Document all patient encounters accurately and timely in the electronic medical record to support care coordination, compliance, and quality outcomes.
  • Participate in case conferences, team meetings, and quality improvement initiatives and support care transitions across settings, including coordination with hospice team

Qualifications:

  • Current Nurse Practitioner license in the state of Michigan required.
  • Possesses and maintains DEA Certification for prescribing controlled substances in accordance with delegation of authority from the collaborating physician.
  • Skilled in symptom management and complex care coordination and working both independently.
  • Flexibility in adapting to the changing needs of patients and families.
  • 2 years of palliative care, hospice, or home care experience required.
