The ideal candidate will assess patients' needs, develop individualized treatment plans, and implement therapeutic interventions that enhance daily living and functional independence.
Responsibilities
- Conduct thorough patient assessments focusing on geriatric needs
- Develop and implement personalized treatment plans
- Provide therapeutic interventions to improve patients' functional abilities
- Maintain accurate and timely documentation
- Educate patients and families on therapeutic techniques and care strategies
- Collaborate with multidisciplinary teams for comprehensive care
- Monitor patient progress and adjust treatment plans accordingly
- Recommend appropriate adaptive equipment and assistive devices
- Coordinate care with other healthcare providers and services
- Ensure compliance with regulatory and safety standards
