Referral Coordinator

Community Health Centers of South Central Texas (CHCSCT)is seeking a dynamic and highly organizedReferral Coordinatorto join our team at our Gonzales Community Health Centerlocation. As a Referral Coordinator at CHCSCT, you will play a crucial role in ensuring that our patients receive the specialized care they need through a efficient referral process. You will be responsible for managing and coordinating referrals, working closely with healthcare providers, insurance companies, and patients to ensure a seamless and timely access to healthcare services. Travel Required

Key Responsibilities:

• Verify patient insurance coverage and eligibility for the referred services.
• Communicate with patients, providers, and insurance companies to ensure timely and accurate processing of referrals.
• Communicate with patients, educate on the referral process, schedule appointments with referred specialists, and answer any questions and concerns regarding referrals.
• Obtain necessary authorizations and pre-certifications for specialist visits or procedures.
• Collaborate with healthcare providers to obtain necessary medical records and documentation referrals.
• Maintain accurate and up-to-date records of referrals and follow-up actions.
• Ensure that referrals are processed in compliance with healthcare regulations and company policies.
• Monitor and report on referral performance metrics to identify opportunities for improvement.

Qualifications:

• High school diploma or equivalent.
• Previous clinical referral experience.
• Strong organizational skills with the ability to prioritize tasks effectively.
• Excellent communication skills, both written and verbal.
• Knowledge of medical terminology and insurance processes is a plus.
• Proficiency in using computer applications and electronic medical records.
• Ability to work well in a fast-paced environment while maintaining attention to detail.
• Ability to speak Spanish/English, preferred.
