Job Title: Occupational Therapist (OT) – Skilled Nursing Facility (SNF)

Location: Willows, CA
Schedule: Full-Time (40 hours per week, Monday to Friday)
Employment Type: Permanent

Overview:
We are looking for a compassionate and dedicated Occupational Therapist (OT) to join our team at a Skilled Nursing Facility (SNF) in Willows, CA. This is a full-time, permanent position offering an excellent opportunity for both experienced therapists and new graduates to develop their careers in a supportive clinical environment.

Key Responsibilities:

• Assess patients’ physical, cognitive, and functional abilities

• Develop and implement individualized treatment plans

• Support patients in improving activities of daily living (ADLs) such as grooming, dressing, and mobility

• Monitor patient progress and update care plans accordingly

• Maintain accurate and timely documentation in compliance with facility standards

• Collaborate with physicians, nurses, and other healthcare professionals

• Educate patients and their families on therapy goals and home exercises

Requirements:

• Degree in Occupational Therapy from an accredited institution

• Active OT license in California (or eligibility to obtain)

• Strong communication and interpersonal skills

• Ability to work both independently and within a team
