New Hire Starting Salary Range: $36.05 – $42.17 per hour. (Promotional increases, after hire – see below)

Purpose
The Senior Case Manager / Discharge Specialist functions as a high-level coordinator accountable for assessing complex post-discharge needs and serving as a vital financial and clinical steward for the hospital. Operating in an advanced model where Registered Nurses (RNs) and Social Workers function equally, you will serve as the central point of contact through a patient's treatment journey. Additionally, this expert-level role provides essential training, mentorship, and operational orientation to less experienced clinical care staff.

Responsibilities
  • Holistic Assessment: Complete comprehensive medical, psychosocial, financial, and cultural assessments to outline barriers to discharge, determine self-care capacity, and address social determinants of health.
  • Transition Coordination: Facilitate interdisciplinary teams to execute safe, efficient, and timely discharges, proactively eliminating unnecessary roadblocks to post-hospital care.
  • Multidisciplinary Liaison: Serve as the core liaison between patients, attending practitioners, family caregivers, and post-hospital care facilities to integrate discipline-specific recommendations into an individualized transition plan.
  • Patient Advocacy: Advocate on behalf of patients for service access, insurance authorizations, and the continuous protection of patient health, safety, and rights.
  • Departmental Mentorship: Assist with operational activities, lead departmental process improvement initiatives, and provide direct mentoring and orientation for staff members.
  • Regulatory Documentation: Maintain rigorous clinical documentation within the patient medical chart in strict compliance with ethical standards and healthcare regulatory requirements.
Minimum Essential Requirements
  • Experience: At least four (4) years of direct experience in discharge planning, care coordination, or case management across inpatient, outpatient, or post-acute settings.
  • Professional Pathways (Must meet one of the following):
    • RN Track: Bachelor of Science in Nursing (BSN) required, paired with an active, unrestricted RN License.
    • Social Work Track: Master’s degree in Social Work (MSW) or a related health/human services field required, paired with an active LSW, LCSW, or equivalent professional license.
  • Certification Requirement: Active professional certification in care management (such as CCM or ACM) or a related recognized nursing/social work certification is required.
