- Active California Licensed Clinical Social Worker (LCSW) license required
- Current BLS Certification required
- Must be California 5150 Certified
- Minimum 2 years of recent Social Work/LCSW experience in an acute care setting
- Prior California Dignity Health experience is required
- Experience with Cerner EMR preferred
- Strong knowledge of crisis intervention, psychosocial assessments, discharge planning, and care coordination
- Excellent communication and interdisciplinary collaboration skills

- Care Coordination white lab coats provided by the unit
- Candidates must be eligible to work in a fast-paced acute care environment and demonstrate strong patient advocacy and care coordination skills.
