Job Details

• Shift: Day Shift
• Schedule: 5x8
• Experience Required: 2 Years
• Weekly Pay: $2300 to $2500

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans
• Perform comprehensive patient assessments and ongoing evaluations
• Administer medications, wound care, IV therapies, and other prescribed treatments
• Monitor patient progress and identify changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Coordinate care with physicians, therapists, case managers, and interdisciplinary healthcare teams
• Develop and update patient care plans based on clinical needs
• Maintain accurate and timely documentation in accordance with agency and regulatory requirements
• Promote patient safety, independence, and quality of life

Requirements

• Active RN License in North Carolina
• Minimum 2 years of recent nursing experience, preferably in Home Health, Med Surg, Acute Care, or Community Health
• Current BLS Certification required
• Home Health experience preferred
• Proficiency in patient assessment, care planning, and clinical documentation
• Strong communication, organization, and time-management skills
• Valid driver's license and reliable transportation preferred
• Ability to work independently and manage a caseload effectively
