Hospice colleagues have the humbling responsibility of administering care to patients transitioning to end-of-life care through hospice services. Community Liaisons with home care hospice have the added responsibility of coordinating the daily activities of the hospice services provided in the home setting to ensure high-quality care is provided to patients utilizing the services administered by HSHS home hospice teams.
Bachelors - Nursing - Preferred
1 Year - Related Experience - Required
Certificates, Licenses & Registrations
Licensed in the state of practice - Required
Valid Driver's License - Required
Driver's License, insurance and vehicle for daily transportation is required.
