    Job Responsibilities include, but are not limited to the following:

    Receive and process telephone orders

    Handle customer service on the phone, in the store and at delivery locations

    Prepare all of our food products

    Stock and prepare ingredients

    Clean the store and dishes

    Marketing and promotions

    Deliver products by car safely to customer's homes and workplaces

    Delivery Expert Requirements:

    You must be at least 18 years old

    Have a valid driver's License for at least one year

    Keep a good driving record

    Maintain a reliable, insured vehicle

    You must be able to perform the essential duties of the job with or without accommodations.

    PSI Pizza, Inc. T/A Domino's Pizza, hereafter known as "Team NEPA", intends to make an independent investigation of your background which may include references, character, past employment, education, credit and consumer information, driving history, criminal or police records, or insurance claim records, including those maintained by both public and private organizations and all public records for the purpose of confirming the information contained on your application and/or obtaining other information with may be material to your qualifications for employment (the "Background Investigation").

    CONSENT: I hereby authorize Team NEPA as part of the application process, and from time to time during my employment, to conduct:
    • A Background Investigation not limited to a consumer report, background report and/or a Motor Vehicle Report.

    I authorize the release of all such information to Team NEPA. I release, without reservation, Team NEPA and any person or entity which provides information pursuant to this authorization, from any and all liabilities, claims or causes of action in regards to the information obtained from any and all of the above reference sources used. I acknowledge that this is a stand-alone consumer notification informing me that a report will be requested and that the information obtained shall be used solely for the purpose of evaluating me for employment, promotion, reassignment, or retention as an employee. I understand the alcohol/drug screens may detect the presence of prescription drugs; therefore, it is important for me to disclose any prescription drugs I am using, or which I have used recently. I authorize Team NEPA to contact the physician who prescribed medications reported by me. Further, I authorize the physician to provide information to Team NEPA relevant to the reason for such prescription and information about its potential effect on my performance.
