    Responsible for identifying patients who are at risk for readmission within 30 days.# Interviews patient and family to assess the reasons for readmission.# Assesses the coordination of patient#s care across the continuum and collaborates with inpatient and outpatient caregivers to optimize continuity.# Coordinates resources at discharge.# Assesses patient and family#s health literacy and readiness for managing their illness.# Educates patient and family.# Orients new team members.# Position requires 24-hour accountability.# May interact with patients or customers ranging in age from newborn to geriatric. Education # Training:# Candidate must have MSW or Masters in Psychology.## Maryland licensure at the LCPC or LCSW-C required.# Will consider LGPC or LGSW with 1-3 years# experience working with acute mental health population in clinic or hospital settings.# Training in aggressive patient management required. Work Orientation # Experience:# Inpatient or Outpatient care experience required.# Case Management experience preferred.# Basic computer skills required.
