This position is based out of Community Hospital South. It involves consultation, communication, and discharge planning, with less bedside care.
The Home Care Registered Nurse is responsible for case management of a group of home health care clients, providing direct nursing care, and coordinating supervising other disciplines involved to promote positive health outcomes and quality services to patients. Responsible for providing skilled care to assigned patient according to the plan of care established by the patient's physician. Practices within the scope of nursing as defined by the Nurse Practice Act.
•One year nursing experience required
•Home care/hospice experience preferred
Muse be a graduate of the Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited program. Bachelor of Science in Nursing preferred.
