    TeleHospitalist for Virtual Care Team located in Menomonee Falls, WI

    Virtual Core Team (VCT) is a remote monitoring team who support various patient populations, most often for off-hours cross-coverage. VCT will serve as first-call for patient care needs.

    •Remote monitoring support. Includes intensivist consultative support, dialysis support, RN consultative support and radiology for the Froedtert & MCW health network
    •Virtual rounding. Twice-daily "virtual rounding" occurs between Virtual Core Team RNs and select SNF sites specific new admissions. "Virtual Rounding" is in place for first 5 days of admission, with an opportunity to extend.
    •ProviderWisdom. The digital platform distills insights by obtaining real-time, real-world insights from clinicians and staff to enhance engagement, reduce burnout and streamline care.

    We are flexible and will consider an FTE from full-time to OPT in order to best fit your schedule. All shifts are 3rd shift.

    *1.0 FTE is Full-Time, any position less than 1.0 FTE is Part-Time, and .001 FTE is Optional Part-Time*

    *Positions with an "FTE" (full time equivalency) of .875 - 1.0 are eligible for full-time medical, dental and vision benefits. Positions with an "FTE" of .5 - .874 are eligible for part-time pro-rated medical, dental and vision benefits*

    Shift 3 / Nights

    Responsibilities:
    The IH Digital Health Nurse Practioner (DHNP) functions as a member of the IH Digital Health Core Clinical Team. Under the direction of a physician, and in collaboration with the remote-monitoring team, the DHNP demonstrates expert skill in the assessment, diagnosis and treatment of complex medical problems across the care continuum through the effective use of a variety of digital health platforms. The DHNP integrates professional experience, education and leadership experience to maximize care and promote innovation. Other duties as assigned.

    Summary:

    A minimum of 3 years of Critical Care Provider experience is required. A minimum of 5-10 years of Critical Care Provider experience is preferred. Experience working with EPIC electronic medical records is preferred. Experience working within the digital health environment/telemedicine (VICU (eICU), Virtual Clinic, etc.) is preferred. A Master's degree from an approved Acute or Adult Nurse Practitioner Program with certification which meets ANCC or AANP requirements is required. Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Current license to practice as an Advanced Practice Nurse Prescriber (APNP) in the State of Wisconsin is required. Certification by the America n Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP) is required. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required. Medicaid and Medicare (PECOS) enrollment is required. DEA Registration is required.

    Other information:
    • Demonstrated ability to apply evidence-based clinical knowledge into the patient care setting via protocols, standards, and outcomes measurement. • Excellent verbal and written communication skills. • Ability to function independently and as a team member. • High level of self-motivation to stay current in rapidly changing environments. Experience working with EPIC electronic medical records is preferred.
