Who Is The Virtual Care Team?
The Virtual Care Team is a centralized team of highly experienced registered nurses, advanced practice providers and critical care intensivists well-versed and successful in integrating technology and innovation into care models to drive optimization. This team is able to leverage their unique perspective to provide system-wide surveillance of data and warnings, partner with the care team in proactive intervention, and lead innovative program adoption, execution, and impact monitoring.
What is a TeleHospitalist?
The TeleHospitalists, a team of MDs and APPs, are positioned to help take care of routine requests to allow the on-site Hospitalist to address the critically ill and attend to their new admissions. Many of these requests frequently fall into the categories of electrolyte supplementation, pain management, and sleep promotion. However, the TeleHospitalist can remotely monitor and assess patients' status through the use of technology, allowing them to clinically manage increasingly complex patients, such as those suffering from severe sepsis, cardiac dysrhythmias, delirium, and cerebral vascular events. In their role as the TeleHospitalist, they can educate team members, provide clinical bedside resources, and proactively round on patients.
