SIGN ON BONUS $3,000 SUMMARY OF JOB RESPONSIBILITIES:
Provide leadership, administration and support in assigned area(s). To ensure achievement of Hospice of the Valley's strategic and operational plans by effectively managing human, financial and capital resources within the spirit of its mission and values. Assumes responsibility for coordinating all available resources to assure safe, appropriate care and treatment for all patients/families needing hospice services; acts as practitioner, role model, educator and change agent within their team. Works collaboratively to plan implement and evaluate practice standards within the team/agency. Works with the director to evaluate practice standards and provide problem resolution using the interdisciplinary team approach. QUALIFICATIONS:
Certification in Hospice and Palliative Care expected within 2 years of appointment to position.
Maintains current CPR certification.
Knowledge related to electronic medical records. EDUCATION
Graduate of an accredited school of nursing and current RN licensure in the State of Ohio. BS/BSN required or attainable within two years of appointment to position(waived for incumbants).KNOWLEDGE AND SKILLS:
Has in-depth knowledge of the hospice/palliative philosphy of care.
Analyzes and interprets complicated information.
Determines a course of action based on general guidelines.
Incorporates understanding of regulations/protocols within area of responsibility. Problem Solving/Decision Making:
Makes decisions based on an analysis of facts and general principles.
Receives guidance through broad policy only and has varied courses of action available.
Requires risk-taking and judgment to adapt existing philosophy or practices. Scope of Impact/Accountability:
Impacts operation of work area on the team and the agency
Accountable for developing a strategic plan for an area of influence and achieving end results.
May contribute to HOV decision-making process as an information resource. Interpersonal Communication:
Frequently advises others on difficult and sensitive issues.
Requires staff management and delegation skills.
Exercises significant interpretation to identify recommended courses of action.
Requires consistent tact, some persuasion, and listening skills to achieve results. PRINCIPAL JOB ACCOUNTABILITIES:
Responsible for facilitating and coordinating referrals and admissions to Hospice of the Valley.
Inspires, motivates and models supportive behaviors while encouraging ownership for the success for the organization. . Fosters the spirit of servant-leadership by promoting empowerment, collaboration, performance improvement processes, inter-disciplinary work teams and partnership relationships with customers. Maintains current knowledge of Medicare Conditions of Participation for Hospice Care. Creates an environment which fosters professional/personal growth and development. Provides orientation, in-service training and continuing education of staff. Participates in QAPI (Quality Assurance and Performance Improvement) for the agency. Participates in data collection and review of statistics Maintains open and effective communication with Medical Director(s) and other members of the interdisciplinary team in reviewing referred cases. Promotes stewardship of resources while maintaining or improving quality of care. Exhibits and encourages ownership for the success of the organization. Regularly communicates goals, plans and actions encouraging employee input and ownership. Effectively engages staff in two-way dialogue and problem solving.
