    Provides treatment of patients referred for home health phsycial therapy under the direction and supervision of the PT.

    Essential Functions (7-10 statements)

    Provides appropriate treatment of physical therapy patients. Works effectively with patients of all ages in the home and community setting.

    Adheres to the plan of care set out by PT. Reports any appropriate information to the supervising PT.

    Schedules work independently and according to patient's clinical needs and agency policy

    Documents care, progress, goals, contacts, and orders according to policy and regulation

    Serves as a care team member, communicating effectively with patients, families, caregivers, physicians, and third party payors

    Works autonomously without direct supervision in the field

    Uses computerized technology for clinical care effectively after on the job training.

    Provides appropriate patient and caregiver teaching

    Complies with and is knowledgable of hospital and department safety procedures and policies

    Maintains customers rights and interacts with customers in a manner that exemplifies the hospitals mission and core values.

    Department Specific Functions (2-4 statements)

    Considerable driving in any weather and road conditions. Complies with and is knowledgeable in reassessment frequencies

    by supervising PT to meet state and payor guidelines. Is willing to assume additional responsibilities.
