    Job Description

    The Nurse I will be assigned to shift and unit that will meet the needs of the hospital.

    The Nurse I functions under the direct supervision of the Unit Nurse Manager, observes, assesses, plans, intervenes and evaluates outcomes and documents all aspects of patient care. Has broad latitude for the use of initiative and independent judgment. Supervises, counsels and evaluates subordinate personnel and coordinates scheduling within assigned area. Provides primary proactive management for service area during shift with a supportive but clear supervisory manner. Collaborates with other nurses, hospital staff, and House Supervisor to ensure 24-hour continuity of care. Assesses, plans, and directs the implementation of nursing care; performs nursing assessments; functions as a member of the interdisciplinary Recovery Team; provides direct care for individual patients; complies with established policies and procedures of HHSC, Rusk State Hospital, and Nursing Services. Maintains a therapeutic environment. Participates in teaching and directing other personnel; may relieve the Unit Nurse Manager or House Supervisor as needed. Performs other related duties as assigned.

    Must understand that duties may include exposure to blood or other potentially infectious materials.

    Essential Job Functions:
    Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

    1. Assumes clinical supervision of the unit in the absence of the Nurse Manager. Provides primary proactive management for service area during shift with a supportive but clear supervisory manner in order to ensure the effective operation of the unit in the provision of safe, quality patient care.
    2. Coordinates scheduling and shift assignments within assigned area.
    3. Assesses the health/mental status of the patient upon admission and on an ongoing basis and documents assessment, planning, intervention and evaluation in patient record in order to determine priorities and plan therapeutic nursing care. Performs Cardiopulmonary Resuscitation (CPR) and Heimlich techniques correctly and safely according to the specifications of the American Heart Association.
    4. Administers medications as ordered by various methods using the "5 Rights" and transcribes and notes orders related to medication administration in order to provide safe medication administration.
    5. Completes medication administration record, and completes control drug count and record following established policies and procedures in order to comply with federal standards and regulations and ensure accurate and complete documentation of the medication administration process.
    6. Implements special precautions as ordered and explains and documents rights restrictions and implements restraint/seclusion procedures according to departmental policy and established nursing standards in order to maintain patient safety, protect the patient from causing harm to self and/or others and uphold patient rights.
    7. Provides and documents individual/group teaching to patients regarding medications, illness, special treatment, etc. utilizing appropriate teaching/response methods in order to facilitate patient learning.
    8. Exercises a high degree of critical thinking and independent judgment in the decision making process in order to facilitate the delivery of quality patient care.
    9. Utilizes therapeutic communication skills in interactions with patients and interacts with patients in a caring, respectful manner in order to promote the healing process and return of the patient to the least restrictive environment. Maintains a quiet and restful environment to promote patient's rest and sleep by utilizing therapeutic interventions with patients exhibiting altered sleep patterns.
    10. Carries out physician's orders and implements nursing interventions according to acceptable nursing care standards and hospital policies and procedures.
    11. Monitors Treatment Intervention and Preventative Strategies (TIPS) techniques and, in the absence of other staff, implements TIPS techniques in order to ensure the well being of patients and staff.

    Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

    Knowledge Skills Abilities:
    1. Knowledge and/or understanding of HHSC and facility policies, procedures, and record-keeping requirements relating to patient care.
    2. Thorough knowledge of nursing procedures and practices and applicable regulatory and accreditation standards and criteria.
    3. Ability to provide patient care, to train and/or supervise the work of others, and to prepare and maintain records.
    4. Must be physically and mentally able to perform Treatment Intervention and Preventative Strategies (TIPS), Cardiopulmonary Resuscitation (CPR), Heimlich techniques and other related tasks.
    5. Clinical nursing skills and competencies necessary to perform the essential tasks of the position.
    6. Skill in observing and evaluating behavior changes in individuals served in a HHSC setting.
    7. Ability to analyze and interpret data of varying levels of difficulty.
    8. Ability to read, write, and understand English so as to effectively communicate with patients, families, and subordinate staff.
    9. Knowledge of the laws and ordinances governing public health and hospital operations.

    Registration or Licensure Requirements:
    1. Licensed as a Registered Nurse in the state of Texas or in a party state that recognizes reciprocity through the Nurse Licensure Compact as described in Art. 4528b of the Nurse practice Act or a valid permit to practice in the State of Texas, while awaiting licensure.

    Initial Selection Criteria:
    1. Must be enrolled in an accredited Registered Nurse program with an expected graduation date, or must have a graduate nurse permit, or a license to practice professional nursing in the state of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact as described in Art. 4528b of the Nurse Practice Act.

    Salary Note: The salary offered will follow HHS starting salary guidelines which are typically made at or near the posted minimum of the salary range.
