    Merakey is looking for a Full-time, Behavioral Health Navigator to join our Bethlehem, PA team

    POSITION REQUIREMENTS
    • Must possess a Masters' degree in Social Work, Psychology, Nursing, Counseling or related field of study.
    • Must possess a minimum of 3 years of direct mental health care experience.
    • Experience in Motivational Interviewing, co-occurring disorders, trauma informed care or other related areas preferred but not required.
    • Bilingual preferred, but not required.
    • Must possess a valid PA Driver's License and access to reliable transportation.
    POSITION OVERVIEW
    • Provide integrated service coordination for an assigned group of consumers, including coordinating and monitoring behavioral and physical health care
    • Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the consumer and any natural or professional supports they identify.
    • Provide initial and on-going assessment of consumer's mental health, physical health, and substance use and response to treatment
    • Coordinate with pharmacy on behalf of consumer and ensure consumer is picking up and taking medications as prescribed.
    • Physical and Behavioral Health support and education to consumer about his/her health conditions.
    • Coordinate services with other members of the Wellness Recovery Team as needed
    • Provide individual and group therapy/treatment in office and community settings
    • Attend supervision and team meetings, trainings as assigned. Follow all Merakey policies and state regulations
    • Administer, review, and analyze Smart Screener as required.
    • Develop a Wellness Plan, Crisis Plan, or Safety Plan with the individual as necessary
    • Complete Wellness Tracker, Health and Wellness Questionnaire (HWQ) and other Wellness Focused Assessments
    • Engage with external and internal stakeholders and educate on the purpose of the Wellness Recovery Team.
    • Discuss discharge planning at intake to determine what is needed for the consumer to meet his/her goal without support from the WRT in the future.
    • Collaborate and communicate with other healthcare staff involved in consumer's care. Complete and coordinate necessary referrals. Provide follow up to assure that recommended services are provided.
    • Ensure consumer's attend/keep medical and psychiatric appointments
    • Ensure consumer's engagement in preventive care (yearly physical health exams, mammogram, colonoscopy, dental exam, eye exam, vaccines)
    • Ensure consumer has access to Mental Health, Substance Abuse, Primary Care, and Dentistry Services
    • Review member profile, psychosocial and medical history
    • Ensure consumer signs consent for treatment and releases of information for coordination of care at intake and on an ongoing basis.
    • Be the liaison between family and other natural supports and provide information and education as permitted by consumer.
    • Coordination with community and inpatient physical health and behavioral health professionals to ensure consumer status is shared upon inpatient admission and discharge.
    • Assist the client in obtaining and maintaining basic living needs and skills including housing, food, medical care, recreation, education, and employment. If additional support is need; make the recommendations and referrals
    • Complete all needed documentation in a timely manner as defined by regulations, Merakey policy, and program guidelines. This includes all assessments, releases of information, consents, and progress notes for all completed and attempted interactions with the consumer, on behalf of the consumer, or for coordination of the consumers care.
    • Monitor insurance per caseload to ensure proper billing
