    Responsibilities

    Job Summary / Purpose:

    This national role develops effective partnership and engagement strategies with internal and external stakeholders on behalf of Enterprise Population Health and oversees pilot programs to test and deploy innovative models. Partnering primarily with the Strategic Innovation and Community Health teams, as well as Division leadership, the System Director will represent Population Health in designing pilots and partnership agreements that further the goals of innovating methods to deliver value based care for the enterprise. The System Director develops and maintains a solid understanding of market characteristics across the enterprise in order to identify the optimal locations and teams for new partnerships and emerging programs. The System Director will direct and guide program and project staff in carrying out pilots, evaluating programs and operationalizing processes.

    The System Director leverages population health knowledge and initiative implementation skills to work effectively with clinical and operational stakeholders to synthesize program information, develop and deploy measurement tools and oversee timelines, budgets and return on investment (ROI) analysis. The System Director leads/facilitates meetings and committees, directs the data analysis and reporting, monitors program performance and looks ahead to mitigate risks. Ensures clear definition of initiative success measures, actively collaborates on financial analysis and is able to communicate about initiatives effectively to a variety of audiences and stakeholders. Reports to Enterprise Population Health leadership and interfaces with senior leaders and team members across the enterprise.

    Essential Key Job Responsibilities:
    • Leads the planning and day-to-day implementation of pilots and external partnerships that promote Population Health innovation.
    • Takes high level goals and provides definition, structure and phasing to achieve through program activities.
    • Collaborates with cross functional resources to develop dashboard for KPIs related to key initiatives.
    • Develops guidelines and toolkits for local implementation by conducting research, supplementing with data analysis, and designing and structuring the material.
    • Partners with Finance to develop, track and report the financial impact of initiatives and works with local partners to determine how to operationalize successful pilots to ensure goals are met over the long term.
    • Builds an effective forum for engagement of local stakeholders in pilot programs, translating potential benefits and finding innovative ways to implement programs in an environment of constrained resources.
    • Leads status reporting and preparation of materials for senior executive audiences, including the Board and Executive Leadership Team.
    • Guides matrixed teams of program managers and analysts to carry out geographically dispersed initiatives

    • Advanced Degree
    Knowledge, Skills, Abilities and Training:
    • Expertise in leading complex initiatives that are new to stakeholders.
    • Proven ability to work with external partners of varying size and ensuring they can work effectively in partnership with CommonSpirit Health
    • Experience in forming partnerships and familiarity with the types of models that work well with a large health system.
    • Financial acumen and skill in developing return on investment models
    • Knowledge of Population Health and value based payment models
