    Job Description

    The Case Manager provides care coordination for a patient#s hospital stay. Working with the healthcare team, the case manager monitors the LOS, the appropriate utilization of care, promotes quality, provides clinical review for the payers, and plans/implements the discharge. The Case Management Department upholds the current standards of case management practice and creates an environment that promotes the individual growth and development of its staff. Responsibilities: Manage a daily caseload which is specialty-based, in accordance with hospital/departmental policy and procedures and regulatory requirements. Optimize their work to accomplish the goals and strategic initiative identified for each fiscal year# This role is carried out in collaboration with the social workers, considering the care and psychosocial needs of both the patient and family. Foundation for this position will include; Integration of the best in evidence-based practice, professionalism and customer service Prepare reports to inform the case managers and department of their outcomes and assists them in evaluating their practice and how to accomplish next steps Requirements: Bachelor#s degree in Nursing is required Current Massachusetts registered nursing license required Ability to work with case manager technology and clinical leveling criteria Able to learn and expand skill set as new protocols and projects are initiated Possess the ability to transfer clinical knowledge/skill to the care coordination of their patients/families Utilize critical thinking to case manager and implement a safe, appropriate discharge Prior experience in case management or a related field is required#",
