Join the Genesis nursing team as a RN Care Manager at one of our PowerBack Rehabilitation centers. PowerBack Rehabilitation centers meet the needs of post-acute patients by providing skilled nursing, medical and rehabilitation therapy, and highly personalized care plans designed to get patients home sooner. As a RN Case Manager, you will lead the primary care teams and are responsible for the care management of the patients' assigned to those teams and for providing the physical care that requires the expertise of a Registered Nurse.
Organize, assign and delegate aspects of patient care to the team for the assigned group of patients, focusing on the prioritized, individualized needs of the patient and the competencies of the team members.
Mentors and assists primary team members providing patient care under his/her direction.
Communicates with assigned patients and families to determine patient needs and concerns.
Provide care to assigned patients, performing skilled interventions that are required by an RN
Completes scheduled patient assessments (MDS, Risk etc.) based on data collected.

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1. Must be a graduate of an accredited school of nursing, college or university with a current registered nurse licensure by the State Board of Nursing. 2. A minimum of two years full-time or equivalent clinical experience is required and a minimum of one years of clinical experience in long-term care nursing is preferred. 3. BSN preferred 4. This position requires that the employee is able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients and visitors at the work site when responding to their medical and physical needs. 5. Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy. TSTs will be administered at the work site if required. 6. Maintains current BLS/CPR certification (All SNFs and Behavioral Centers; ALFs/ILFs as required by State).
