Assertive Community Treatment (ACT / FACT) Teams are composed of professionals including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services including therapy, medication management and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week.Responsibilities:
The Psychiatric Nurse Practitioner provides medication management and directs the program of the Assertive Community Treatment Team
, a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified clients with severe and persistent mental health illnesses: supervises and evaluates the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful and respectful services to program clients; and functions as a practicing clinician on the team.
- Adult Psychiatric Mental Health Nurse Practitioner, PMHNP or Psychiatric Mental Health Practitioner required with prescriptive authority.
- Experience with Integrated Health preferred.
