    Job Description

    Merakey is seeking a Community Based Drug and Alcohol Case Manager to join our Behavioral Health Division


    Responsibilities include but may not be limited to:

    Orient client to the knowledge of addiction as a disease and its treatment.

    Develop and implement a comprehensive service plan based on the results obtained in the assessment process.

    Update service plan as needed.

    Assesses and evaluates new target population adults by locating and linking to the appropriate resources.

    Serve as an advocate for clients

    Prepares and completes all regulatory mandated paperwork and reports, as required.

    Participates as a team member of the clinical team.

    Monitors service delivery by maintaining regular contact with the client, family, and service provider.

    Attends scheduled meetings on a regular basis.

    Maintains CQI requirements.

    Coordinate and provide access to available drug and alcohol and ancillary services such as benefits, housing, childcare, medical, mental health, transportation, legal, and employment services.

    Plan and organize work, prepare record, set priorities and maintain a caseload in an effective and timely manner with limited supervision.

    Maintain client charts with accurate and complete paperwork, file all client notes, and perform weekly chart maintenance.

    A Bachelor's degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or related field; and 2 years direct service experience performing direct support, counseling or case management work in a drug and alcohol and or co-occurring setting.
