    The Licensed Vocational Nurse performs a range of assigned nursing tasks and procedures under the supervision of a Registered Nurse.

    Primary Responsibilities:
    • Assists, contributes and participates in the nursing process by performing a focused assessment on individual patients over the telephone to collect data and gather information
    • Implements aspects of the nursing care plan over the telephone, including emergency interventions under the direction of an appropriate clinical supervisor
    • Over the telephone, contributes to the development and implementation of teaching plans for patients and their families with common health problems and well-defined health needs
    • Acts as a float in the call taking areas facilitating clinical questions for non-clinical personnel, while also participating in scheduling internal and external referrals, office appointments, lab and other diagnostic testing
    • Prepare patients by answering questions over the phone regarding examinations and treatments
    • Perform phone assessments to determine the patient level of care needed. The patient may be able to supply values such as (i.e. weight, blood pressure, medical history, health maintenance and chief complaint) to aid in the telephonic assessment. All information obtained will be documented as a communication message into the patient's EMR records
    • Triage and process telephone and e-mail messages, as well as medication refill requests, from patients and provide feedback and answers to patient/provider/pharmacy per physician protocol. Assignment of tasks to physicians, associate providers and clinical staff accordingly
    • Capture, document, and authorize non-narcotic maintenance dosed medications in accordance with clinical standards and physician orders

    Required Qualifications:
    • High School diploma or GED
    • Current unrestricted LVN license (specific to state of employment)
    • CPR and / or BLS Certification
    • 2+ years of nursing experience in a clinic environment
    *This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease.

    Physical Demands & Working Conditions:
    • Ability to frequently to sit, stand, walk, see, speak, and hear
    • Ability to occasionally kneel, climb, or balance
    • Ability to regularly required to lift up to 10 pounds and rarely lift more than 25 pounds
