    Job Summary:

    The PPS Coordinator, in conjunction with the units Medical Director and Nurse Manager, coordinates prospective payment system.

    Essential Functions:

    Performs duties of the Rehab Clinical Assessment Coordinator as required.

    Completes inpatient Rehab Facility Patient Assessment Instrument (IRF-PA)

    Actively participates on the Referral Development Team.

    Provides FIM training for staff.

    Assures completion of FIM's at admission, discharges, and follow up. Accurately tracks and reports patients' FIM scores.

    Acts as a liaison for the team and medical records and finance personnel to ensure accurate documentation, coding and payment.

    Complies with Medicare guidelines on data entry and exporting files within the defined timeline as outlined in the Federal Register.

    Reviews medical records to ensure appropriate documentation occurs to support findings noted on the IRF-PA. Monitors medical necessity and three hour therapy rule.

    Serves as an education resource to patients, families, interdisciplinary team members, payor representatives, physicians, case managers, community members, and other parties as appropriate regarding rehabilitation issues.

    Assists with internal and external marketing of the service.

    Tracks and outcomes for FIM accuracy and change, LOS efficiency, and 60% rule compliance.

    Works closely on a day to day basis with the interdisciplinary team as a communicator, coordinator, program evaluator and educator.

    Works with Medical Director and Interdisciplinary Staff or to ensure the pgram meets all local, state, Joint Commission and CARF regulatory standards.

    Compiles outcome measurement data and submits to Uniform Data System (UDS).

    Assists with the review of potential admissions to the Rehab and TCU units under the guidance of the Admission Coordinator. Works with physicians and case managers to facilitate proper placement of patient in the continuum.

    Completes necessary reports from the software program to assure ongoing monitoring of unit performance.

    Complete and compile monthly medical record chart audits for performance improvement indicators. Monitor and maintain working knowledge of rehabilitation regulations specific to Inpatient Rehabilitation Facilities and inform.

    Minimum Education:

    Required Minimum Education: 2 year/ Associates Degree.Specialty:Nursing

    Preferred Education:4 year/ Bachelors DegreeSpecialty:Nursing

    Minimum Qualifications:

    Minimum Years and Type of Experience:Five (5) years experience as an RN with experience in an Acute Rehab preferred.

    Other Knowledge, Skills and Abilities required:Analytical, communication, computer skills. Must have comprehensive knowledge of Medicare regulations for IRF-PPS.

    Other Knowledge Skills and Abilities preferred:Working knowledge of Functional Impairment Measure (FIM) required.
