Under supervision of director of pharmacy, clinical pharmacy manager, and pharmacy operations manager, the Medication Management Pharmacist will conduct patient visits and be responsible for management of medication regimens of patients enrolled in the clinic. The Pharmacist will also be responsible for precepting post-doctorate residents and doctor of pharmacy students training in ambulatory care.
The Medication Management Pharmacist oversees all clinic personnel including other clinical pharmacists, pharmacy residents, nurses, students and auxiliary staff.
NATURE AND SCOPE:
This position interacts with all clinic personnel including other clinical pharmacists, pharmacy residents, nurses, students and auxiliary staff, the director of pharmacy, clinical pharmacy manager and pharmacy operations manager.

Documents renewal of license on a timely basis.
Precept post-doctorate residents and doctor of pharmacy students during ambulatory care rotations.
Be an active member of residency advisory board and other committees as needed.
Conduct patient visits. Use CoaguChek S and XS Plus and Cholestech LDX point-of-care machines. Gather capillary blood sample.
Gather all information needed to adjust drug regimens according to lab values and patient-specific factors. Document all patient care activities.
Communicate regularly with referring physicians.
Maintain inventory of enoxaparin, Vitamin K and other medications as needed.
Assure integrity of patient charges.
Use appropriate resources to provide drug information and education.
Educate patients regarding drug therapy and lifestyle modifications related to their specific condition.
Attends in-services and is compliant with essential education without prompting from manager.
Performs other duties as assigned.
Understands and can use references related to the field of pharmacy.
Willingness to work with realization that errors may have serious consequences for patient.
Considerable initiative and judgment involved in solving pharmacological problems, requisitioning supplies and providing information to physicians and nurses.
Excellent organizational and time management skills.
Excellent verbal and written communication skills.
Is familiar with federal and state laws as well as public health matters.
Pass annual competencies specific to the pharmacy department and job duties.
Licensed as required by law for practice of Pharmacy, by the State of Florida.
Maintain continuing education as required by the Board of Pharmacy.
Certified Immunization Provider.
Compliance with continuing education regulations for Florida licensure.
Must have Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree.
Must have completed PGY1 residency program with significant experience in ambulatory care or have other equivalent professional experience.
One (1) to two (2) years' experience in ambulatory care clinic or equivalent experience required.
