    Position description

    The Hospice Patient Care Coordinator (Registered Nurse) is accountable for the delivery of nursing services based on initial and ongoing assessments of patient needs and in accordance with the interdisciplinary team plan of care. The delivery of nursing services includes direct care, patient/family/caregiver support, instruction and education. In addition, the RN provides telephone triage and on-call responsibility 24/7. The ANA Standards of Clinical Nursing Practice and the Standards of Practice for Hospice Programs provide a basis for the practice of the RN in hospice. The RN supports professional nursing practice across practice settings by providing holistic, family-centered care in efforts to improve quality of life.

    Qualifications

    Graduate of an Accreditation Commission for Education in Nursing (ACEN) accredited associate's degree or diploma program. Exceptions to this may be made in the MCHS sites prior to a law change requiring accreditation expected in 2018. If graduated from a nursing program that was not accredited by ACEN, at least one year of RN experience (within the last 5 years) in an applicable care setting is required. If graduation did not occur within the last two years, one year of RN experience in an applicable care setting is also required. Rochester campus: All entry-level associate degree registered nurses with a RN start date after September 1, 2010, must provide documented evidence of program completion of their baccalaureate degree in nursing from an accredited nursing program within 6 years from the last day of the month of their RN start date. All entry-level associate degree registered nurses with a RN start date of September 1, 2014, and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within 5 years from the last day of the month of the RN start date.

    Additional qualifications

    Minimum of two years previous RN experience in acute care, home care and/or long-term care settings preferred. Demonstrates strong verbal and written communication skills. Possesses ability to adapt to unpredictable situations within the home setting Demonstrates computer and keyboarding skills preferred.

    BSN preferred.

    Internal applicants should submit their three most recent performance appraisals.

    License or certification

    Must obtain prior to start date:

    Current RN license by applicable state requirements.

    Basic Cardiac Life Support for healthcare providers- BCLS - certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

    We will only accept certificates of course completion from the following providers:

    •American Heart Association (Title: BLS for Healthcare Providers or BLS Provider)
    •Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
    •Military Training Network

    Drivers License and Proof of Auto Insurance

    Additional specialty certification/training as required by the work area.

    Compensation Detail

    Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $34.72 per hour.
