    Job Description

    Supports physician in provision of patient care with accountability for designated patient caseload. Facilitates pre-certification and payor authorization processes; collaborative management of patient care across continuum, intervening as necessary. Plans to meet patient needs to ensure high quality of patient care, reduced length of stay, efficient utilization of resources and appropriate reimbursement of care and services. Applies process improvement methodologies in evaluating outcomes of care. Supports Clinical Documentation Improvement Program (CDI) by performing medical record reviews, identifying current DRG designation, ensuring documentation accurately reflects acuity. Serves as clinical resource for medical records coders and coordinate communications with physicians.

    Three to four years experience with one year recent clinical experience in specialized area required.
