    Summary of Primary Function
    The Stroke Program Coordinator ensures quality, timeliness and effectiveness of stroke care delivery for the West Market and collaborates with the Core Stroke Team for program assessment, planning, organizing, implementation, budgeting, performance improvement, education, stroke program hospital services utilization, community outreach, pre-hospital education, and marketing/communications. The Stroke Program Coordinator works in a multidisciplinary role to promote quality in clinical practice for a specific patient population with the goal of providing superior quality, competitive value, and outstanding service to patients, families, and the community.

    Essential Job Functions
    • Coordinates West Hospital Stroke Program in collaboration with the Stroke Program Medical Director
    • Works collaboratively with the medical staff and hospital departments to assure there are effective Stroke Care services in place
    • Acts as chair or facilitator of committee and/or teams
    • Maintains the Stroke Program PI Plan in conjunction with Quality Department
    • Collect data and oversee processes for data collection. Aggregate and analyze data. Provide Benchmarks. Verify Statistical measures. Understands data bases and data collection methodologies specific to the stroke program.
    • Responsible for compliance of the Joint Commission Disease Specific Standards (PSC)
    • Responsible for initial and ongoing education programs and development of content for Stroke Program caregivers in compliance with mandatory standards
    • Functions as a role model on best practice initiatives in stroke and neurological nursing care. Maintains responsibility for knowledge and skill of clinical requirements in nursing specialty.
    Certifications
    BLS Basic Life Support - American Heart Association (preferred)
    Valid nursing license from the state of practice (required)

    Stroke Certified RN (SCRN) within 1 year of hire

    Education
    Bachelors of Science, nursing (required)
    Graduate degree, nursing (preferred)

    Minimum Qualifcations
    Minimum Years and Type of Experience:
    Four years of nursing experience as an RN
    Excellent knowledge and understanding of stroke patient care
    Other Knowledge, Skills and Abilities Preferred:Previous project management preferred
    Previous nursing education experience preferred
