UNC's Department of Pharmacy is an organization of more than 300 dedicated employees with focused attention on caring for patients of the UNC Health Care system. The department is culturally diverse and has a variety of backgrounds, interests, capabilities, and experience. Additionally, the UNC Department of Pharmacy is nationally recognized for excellence in pharmacy practice and has received numerous ASHP Best Practices Awards in Health-System Pharmacy.

For over 40 years, clinical pharmacy services have played a vital role in the multidisciplinary management of hematology/oncology patients at the UNC Medical Center. The Department of Pharmacy Hematology/Oncology Team provides comprehensive care for patients with a variety of hematologic malignancies and disorders and solid tumors in both the ambulatory care and acute care settings with an emphasis on continuity of care.


The focus of this position will be providing evening clinical services, including comprehensive pharmaceutical care, disease management, and transitions of care in the acute care setting to adult patients on the inpatient hematology/oncology services, as well as a couple of medicine services. This individual will ensure continuity of care as patients transition from the inpatient to the outpatient setting and vice versa, while continuing to pursue best practices in these areas. This individual will also be responsible for providing experiential instruction to Doctor of Pharmacy Students and UNC Medical Center pharmacy residents in support of the Partnership in Patient Care.


This individual will directly report to the Clinical Manager of the Hematology/Oncology Service Line, but oversight will involve a close collaboration with the Clinical Manager of Acute Care Services. Additionally, this individual will work closely with the Evening Clinical Pharmacist Team, the Hematology/Oncology Clinical and Central Specialists, the Acute Care Clinical Practice Team, Sterile Products Area, Central Inpatient Pharmacy, Central Outpatient Pharmacy, and Medication Transitions Specialists.


The primary job responsibilities include providing comprehensive pharmaceutical care and optimizing medication use for patients on the inpatient hematology/oncology services as well as providing transitions of care coordination. Clinical services and pharmaceutical care will also be provided to medicine services covered by this evening position. In addition, the selected candidate will serve to develop innovative clinical services and participating in quality improvement initiatives. This position will also include weekend and holiday rotations.

Effort allocation will be as follows:

Evening Clinical Service responsibilities (80%)

  • Provide support to medical and nursing staff, including order verification, profile review, chemotherapy order review, clinical consultation as needed, and ensuring appropriate and optimized medication therapy management for patients on covered services
  • Providing drug information to interdisciplinary colleagues
  • Work on quality improvement initiatives within the Department of Pharmacy and within the Cancer Center
  • Complete therapeutic drug monitoring (e.g., immunosuppression, antibiotics, and anti-fungals)
  • Review all chemotherapy orders for accuracy and appropriateness prior to Beacon order verification by Cancer Hospital Infusion Pharmacy (CHIP)
  • Collaborate with physicians to optimize orders,
  • Ensure hospital wide adherence to chemotherapy policies and practices
  • Assist with transitions of care
  • Discharge medication reconciliation
  • Admission medication reconciliation
  • Coordination of prescriptions with outpatient pharmacy (facilitate discharge medications through UNC COP when possible)
  • Provide discharge counseling when needed and complete appropriate documentation
  • Provide pharmacy support for all rapid responses and codes in the cancer hospital
  • Assist in development and implementation of innovative disease management and medication use programs
  • Service Responsibilities (10%)

  • Develop research initiatives to advance practice in oncology and pharmacy services at the UNC Medical Center
  • Serve as a standing committee member on a select group of committees within UNC Medical Center
  • The pharmacist will contribute to ongoing quality assurance initiatives for the Department of Pharmacy, the Hematology/Oncology Division, and UNC Medical Center
  • Educational Responsibilities (10%)

  • The evening acute care clinical pharmacist will have responsibilities to educate students from the UNC Eshelman School of Pharmacy, PGY1 and PGY2 residents from UNC Hospitals on their experiential rotations.

  • Strong clinical skills and pharmacotherapy knowledge base in Hematology/Oncology
  • Strong oral and written communication, organizational and prioritization skills
  • Ability to work in a team environment
  • Preferred EXPERIENCE

  • Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience
  • Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience
  • Board Certification in Oncology Pharmacy (BCOP)
  • Compensation will be competitive and commensurate with experience.
