Home Instead is one of the largest and fastest-growing home care companies in our community and is seeking a strong, determined, passionate, and innovative to care individual to be our new Client Care Coordinator, for clients in our respective territory. You will join a team of individuals who collaborate and support each other in our goal of providing the highest quality of personal care in the Quad Cities. This is is a self-starting role that will manage the care of seniors and clients in our community. 

Job Summary
We are seeking an individual who can plan and coordinate care based on needs, evaluate the effectiveness of care provided identify and evaluate the significance of social, environmental, and emotional factors relative to identified health concerns. Our Client Care Coordinators are expected to perform a variety of duties in the care for our clients including, care consultations with potential clients, Client/staff introductions, and quality assurance visits with our existing clients. This individual will evaluate the care plan through effective communication and in-person visits to ensure high-quality care, client satisfaction, and increased client engagement.

Job Duties

  • Improve our clients' level of engagement, quality of life, happiness, and comfort 
  • Meet with potential clients and family members to discuss needs and provide solutions develop service plans 
  • Conduct service inquiries and care consultations as needed (consultative sales process)
  • Maintain ongoing communications and a consistent follow-up scheduled with potential clients 
  • Maintain effective communication with team members to coordinate client's care 
  • Conduct introductions with staff and clients 
  • Evaluate and document the effectiveness of the client's care plan 
  • Serve as the liaison between the client and the community, CAREGivers, and staff
  • Participate in the on-call rotation 
  • Other Client Care duties as assigned

  • The ability to work independently to meet challenging goals and deadlines
  • Advanced computer skills, including heavy use of Microsoft Office, Social Media, and Online Job Boards.
  • Creative strategies to attract and engage clients/client families 
Job Requirements
  • Associate's Degree or higher in a related field
  • 1+ year of in-home care, healthcare, or senior-related industry required (an equivalent combination of education & work experience may be considered for the right candidate)
  • Experience working collaboratively within a team of your peers to achieve client care success for the company
  • Must be willing to travel in the community (Quad City area) and surrounding areas. We service both Rock Island County and Scott County.
    • Must possess a valid driver's license, reliable transportation, and valid car insurance
