Summary

This PACT RN position is located in the Primary Care Service at the Worcester (Lincoln Street) location. This position is a Full-Time at 40 hours per week.


Responsibilities

The RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Team (Patient, Primary Care Provider, RN Care Manager, Health Technician/LPN and clerical staff) and expanded team members (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient centered holistic care plan. The RN Care Manager is a member of the Team. The RN Care Manager is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention and wellness, based on the patient's goals. The RN Care Manager demonstrates leadership by serving as an advocate for patients, and role model and team player to colleagues as she/he continues to enhance his/her own and the Team's professional growth, development and practice. Duties include work during off-tour hours.

The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider . The nurse's primary commitment is to the patient, whether an individual, family, group, or community. (ANA Code of Ethics for Nurses). The registered nurse, as a licensed professional, will be responsible and accountable for his/her practice.
