Hospice Registered Nurse RN Case Manager
$5,000.00 Sign-On Bonus
Shift: Monday-Friday 8a-5p
Provides direct patient care under the supervision of a registered nurse (RN). Responsibilities include
following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Education and Experience Requirements
Education:
• Graduate of an approved School of Professional Nursing;
• Current state RN licensure in the state of practice;
• Possess and maintain current CPR certification.
Experience:
• Hospice experience preferred but not required.
