Are you a skilled RN looking to take on a leadership role? We're seeking a motivated and compassionate RN Supervisor to join our team at the MVHS Rehab and Nursing Center!
As the Nursing Supervisor, you'll be responsible for assessing, planning, implementing, and evaluating the nursing care of our residents in our skilled nursing setting. You'll work closely with CNA's, LPN's, and other supportive staff members to ensure continuity of care between shifts. You'll be the resident advocate, always keeping our facility's mission at the forefront.
We're looking for someone with a sharp eye for detail who can document appropriately and notify physicians of any changes in the resident's condition. As the Nursing Supervisor, you'll perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
We want someone who can lead by example and promote excellence in our nursing department. You'll supervise and redirect other members of the healthcare team, as needed, to accomplish the Plan of Care which has been developed.
You'll function as a leader for the CNA's and LPN's by delegating in a manner that contributes to the excellence of the nursing department and maximizes teamwork.

- Assesses residents on admission, readmission as well as when condition changes. Documents appropriately including medical record.
- Demonstrates ability to identify and interpret clinical changes as they relate to medical diagnosis.
- Notifies physician of changes in resident’s condition and follows through until appropriate action is taken.
- Gives a thorough report to oncoming shift and participates in walking rounds.
- Assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.
- Assesses the resident’s condition and nursing needs; sets goals; prescribes appropriate nursing action.
- Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors. Supervises nursing staff to promote this.
- Submits written documentation in timely, clear, concise and pertinent manner.
Exceeds: Identifies and resolves areas for poor documentation solutions are sustained. - Notes and carries out physician’s orders in a timely manner.
Exceeds:No transcription errors in a 12-month period. - Reports any resident, personnel or administrative problem to Director of Nurses or appropriate Department Head and assists in the resolution of problem.
- Follows through, as needed, on information given by shift report, resident or family regarding resident concerns.
- Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.
Required: Two years experience in a medical/surgical or nursing home. One year experience in a supervisory capacity.
Required: Registered Nurse licensure in NYS/BSN or related degree
