Job Summary

Under the direction of the Registered Nurse and utilizing the nursing process, provides care to patients in accordance with their developmental needs, following the standards of the Department of Nursing and in accordance with hospital policy and procedure.

Core Job Responsibilities
  • Provides direct care, under the supervision of a Registered Nurse.
  • Contributes to the formulation of a written care plan for assigned patients.
  • Maintains nursing documentation, including patient history, treatments, responses and assessments of changes on assigned patients.
  • Re-assesses patients, identifying adverse changes in patient condition and notifying persons utilizing the chain of command.
  • Provides skillful, thorough, organized and safe nursing care to assigned patients.
  • Assists with procedures and performs services requiring technical and manual skills.
  • Manages and operates equipment safely and correctly.
  • Performs patient assessment and re-assessment per policy.
  • Collaborates with the Registered Nurse to meet patient learning needs.
  • Reinforces patient teaching related to medications and self-care.
  • Evaluates patient’s understanding of information; including family, in the teaching as appropriate.
  • Contributes to care plan development and revisions.
  • Collects, documents and communicates information on assigned patients accurately and timely per hospital policy and procedure.
  • Communicates both verbally and in writing with members of the healthcare team.
  • Administers medications and observes patient for untoward side effects
  • Participates actively in staff development activities, by facilitating the learning experiences for students and new employees.
Education/Experience Requirements

Required: Current New York State Practical Nursing Registration

Licensure/Certification Requirements

Required: CPR Certification
