This position interfaces closely with the medical staff, fellow department heads and administration, treatment team/external case managers/managed care organizations, functions as a liaison with other hospital intake departments.
Staffs cases with counselors to help determine appropriate level of care and effectively coordinate the efforts of intake.
Establishes effective relationships with customers (patients, referral sources, families, etc.,), ensuring proper customer service is carried out and making follow-up phone calls as needed.
Responds promptly to any customer complaints and follows up per CPH policy.
Audits assessments and call sheets, as well as other documentation as deemed necessary, to ensure that files are complete and all pertinent information is documented.
Ensures an adequate system of communication and reporting is maintained among and between all staff members and other departments involved in the A&R process.
Maintains open, professional relationship with staff to ensure that concerns and issues can be readily identified and addressed per CPH policies and procedures.
Reviews and updates departmental policies as needed, in conjunction with senior management.
