Description:
Judge Healthcare is actively seeking a part-time Nurse Practitioner for Home Assessments with a Healthcare system based in Franklin or St. Charles Counties, MO!

Terms: Part-time, Flexible weekly schedule!

This job will have the following responsibilities: 

  • Conduct in-home assessments on health plan members
  • Check vitals and conduct physical exams
  • Identify needs and opportunities to generate clinical referrals
  • Identify diagnoses to be used in care management
  • Educate members on various topics

Qualifications & Requirements:

  • NP license and national certification (AANP or ANCC) required
  • Minimum of 1 year clinical experience required, home health care or home visit experience is a plus
  • Family, Geriatrics, or Adult certification required
  • Excellent administrative, organizational and verbal skills
  • Reliable transportation in order to complete home visit assessments
