Nurse Navigator Thoracic - Full-time / Part-time

The Care Assure Navigator manages the daily operations and workflows of the Lung Nodule program in conjunction with physician(s) and administrative leadership. The focus of this role will be to manage and follow up on lung nodules found through various avenues including but not limited to high-risk lung screenings, incidental findings in the Emergency Room and inpatient admissions. The Care Assure Navigator will coordinate the next level of care for patients with lung nodules and assist patients, clinicians and program stakeholders in the collection, reporting and presentation of program outcomes.

Job Summary and Qualifications

What you will do in this role:

• Serve as a Patient advocate
• Streamline care coordination and paperwork by assisting the patient with scheduling appointments and exam preparation.
• Support providers and assist patients in understanding their treatment options and resources available.
• Track and document interventions and outcomes
• Drive process improvement

What qualifications you will need:

• High School Graduate / GED, or Associate Degree, or vocational School Graduate
• Associate Degree
• State of Florida LPN or RN License required
• Minimum 2 or more years of clinical healthcare experience
• BLS certification via American Red Cross required
• ARRT (R) (CT) preferred
