One ofTexas'smost highly respected Senior Living Facilities is seeking a Registered Nurse MDS Coordinator to join their growing team. This is an award-winning, financially strong company with a long history of excellence in long-term care.

Responsibilities for Registered Nurse MDS Coordinator:

  • Assess and evaluate the quality of care being given to long-term care residents
  • Perform the initial interview and assess new residents
  • Develop a care plan that has long-term and short-term goals for improvement
  • Conduct meetings with multidisciplinary treatment teams, caretakers, and families
  • Reach out to specialists within the community to get help in achieving care plan goals


  • At least 3 year of nursing experience in long term care or a hospital setting
  • At least 2 years MDS Coordinator experience
  • RN Certification/License
  • CPR Certification is preferred
