RN - Outpatient Clinic $7,000 Sign on Bonus - - Full-time

All's Well Healthcare is seeking a Full-time Registered Nurse to work 40 hours per week, Monday-Friday. Generous sign-on bonus and benefits available.

Our Adult Intensive Services division is made up of several programs that provide daily, specialized support to adults with a severe and persistent mental illness, while maintaining ties to their family and community. Intensive medical services collaborates with therapeutic teams that provide case management, vocational, residential, substance abuse, and therapy services to help patients maintain daily functioning.

Job Requirements:

Associate's in Nursing required. Bachelor's degree in Nursing preferred.
Preferred: one year of relevant clinical experience.**
Licensure: Active State of Colorado Registered Nursing License.
Ability to administer medications and injections.
Effective communication and coordination skills.
Effective organizational skills.
Effective problem solving skills.
Ability to take directives
Basic computer skills and ability to learn/utilize electronic health record.

Job Duties:

Completing all medical orders and communicating vital patient care information to physician/prescriber.)
Perform nursing interventions for which an RN/LPN has been trained to do.
Observe, identify and monitor condition of patient, including changes in symptoms, and relates same to appropriate members of the treatment team.
Administer injections and medications, as required.
Provide medical case management and other clinical interventions, as appropriate.
Act as liaison to outside agencies and providers to ensure continuity of care for patients.
Provide assistance in crisis intervention, as needed. In a timely manner, accurately and completely chart interventions, lab results, medications dispensed and all activities related to patient interactions according to organization's procedures.
Complete required education and training in a timely manner.
Provide information to credentialing, payroll, etc. in a timely manner.
Monitor and coordinate care for assigned clients in partnership with the other members of the client's care team.
