Telehealth Patient Facilitator - Full-time

TELEHEALTH PATIENT FACILITATOR JOB SUMMARY:

• Retrieve any necessary forms and paperwork needed by the Telehealth Provider
• Prepare treatment rooms for appointment
• Obtain patient consent when required
• Ensure all equipment is functional and fully operational
• Assist with flow of patients in and out of Telehealth clinic and remain present during clinic session
• Ensure completion of all required forms
• When applicable coordinate with scheduler for scheduling appointments
• Monitor daily clinic no shows and coordinate with security to facilitate patient arrival, adjust clinic times in response to lockdowns count, etc.
• Other duties as assigned

TELEHEALTH PATIENT FACILITATOR HIGHLIGHTS:

• Work schedule: Mon-Fri, 8AM 4PM
