Licensed Master Social Worker (LMSW) - Full-time

Duties:

The goal of the social worker is to help patients regain optimum health or improve functional capability given the patients setting.

Engage in assessments, create a treatment plan including referrals.

Each day, youll maintain patient medical records and medical documentation consistent with state regulations DocGo standards and policy through our HealthPoint system.

Participate in continuing education as required by the state where you reside.

Other related duties as deemed necessary.

Skills:

Strong organizational skills for collecting data and reporting on performance of the program.

1 year LMSW experience is required.
