***Should reside in Cuyahoga or Lake County, will be member facing in Long Term Care Facilities and Assisted Living Facilities***

Position Purpose: Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.

Daily Responsibilities:

• Reach out to members on cases, pregnancies and education.
• Alert them to problems, contact doctors.

Pay: $35-$45 hourly (determined on experience)

Work Schedule: Will be 40 hours week from 8am-5pm Monday Friday

Required Education & Experience:

• RN active license in good standing with the Ohio state board of nursing is required.
• 1-3 Years of OB experience. (i.e., Post-Partum, Labor Delivery, etc.)
• Knowledge of computer skills: Type at least 45 wpm, and have skills with MS Word, Outlook, & Excel
• Case Management certification (CCM) is a plus.
• Tru Care experience Is A Plus
