Under the general direction and supervision of the Clinical Operations Manager, is responsible for assessing, planning, promoting and evaluating quality patient care. Utilizing the nursing process in accordance with the standards established by the State Nurse Practice Act, Code of Ethics for nurses, ANA Standards of Care and the mission, philosophy, policies and procedures of St. Elizabeth Medical Center. Participates as an integral team member of the Patient Centered Medical Home. Engages in, and assists in the education of Family Medicine Resident Physicians to create an appropriate learning environment, and to promote patient centered team based care. Understands, participates and promotes the nursing role related to patient care documentation, continuity, safety and patient satisfaction and sets the example of performance excellence.
- Adheres to philosophy, goals and standards of nursing practice to maintain high level of patient care in the Patient Centered Medical Home
- Supports initiatives as set forth by the organization
- Prepares the visit agenda based on plan from last visit, triage, or phone message
- Observes, collects and records physical, psychological, environmental and self care ability as well as educational needs;
- Reconciles prescription, OTC medications, and Herbal supplements at each encounter; Prepares refills, includes accessing and documenting iSTOP
- Documents patients allergies at each encounter
- Documents and completes appropriate meaningful use indicators
- Completes any initial assessment according to guidelines
- Evaluates patient’s preventative service requirements and clinical outcome measures,
- Documents the review of the relevant major body systems,
- Documents the review of the patients self care,
- Documents signs and symptoms that indicate risk situations,
- Documentation reflects an ongoing assessment of the patient’s condition at each subsequent visit.
- Proactively collects required health record information needed for comprehensive care of the patient when following up from and ED, inpatient, specialty, Rehabilitation or similar encounter before the visit
- Accesses NYSIS and update immunization history
- Prepares the patient, equipment and supplies needed for the planned care of the encounter
- Assists providers when performing procedures
Required:
- Graduate of accredited school of Nursing.
- One to three years experience as a staff or charge nurse in a unit of expertise with demonstration clinical competence.
Preferred:
- Outpatient/Ambulatory experience preferred
Required:
- Current licensure in New York State as a Registered Nurse.
- CPR/BCLS certification.
