Position Details:




  • Department: Neurology

  • Primary Location: 111 Bedford Road, Katonah, NY 10536

  • Secondary Location: 111 South Bedford Road, Mount Kisco, NY 10549

  • Schedule:

    • Monday (Mount Kisco): 8:30AM - 5:30PM

    • Tuesday - Friday (Katonah): 8:30AM - 5:30PM







Primary Responsibilities:




  • Works with primary care providers and staff throughout the organization to provide education and monitoring services for all referred patients 

  • Assesses patients for their learning needs, follows them appropriately and documents in the patient electronic medical record 

  • Utilizes the curriculum developed for the care of patients 

  • Provides support, teaching, and ongoing management services to patients 

  • Provides self-management education in both group and individual visits 

  • Complies with health and safety requirements and with regulatory agencies such as DPH, etc.  

  • Complies with established departmental policies, procedures, and objectives 

  • Attends a variety of meetings, conferences, and seminars as required or directed 

  • Performs other similar and related duties as required or directed

Required Qualifications:




  • Graduate of an accredited School of Nursing 

  • Currently licensed as an LPN in the State of New York 

  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation





Preferred Qualifications:




  • CPR/BLS certification

  • NextGen EHR experience 

  • Knowledge of various software applications such as Microsoft Excel, Word, Outlook, etc.

New York Residents Only: The hourly range for New York residents is $18.80 to $36.78.
